Valutazione dell'efficienza tecnica e qualitativa delle strutture ospedaliere italiane : analisi DEA a due stadi

The last few years have shown a growing trend in healthcare efficiency measurement. Very often the efficiency measures focus more on the production ignoring the qualitative aspect because of measurement difficulties. The goal of this work is to develop a model for evaluating the technical and qualitative efficiency and understand whether the presence of differences on management, information technology, financial, organizational, affect the value of efficiency. The model was built and refined based on the literature review and on empirical experience. A Survey will show the status of implementation of some of the guidelines of the Patto per la Salute 2014 – 2016 to verify if some of the levers proposed by the survey can positively influence the efficiency. The analysis performed here is in two stages. In the first stage, healthcare efficiency is measured via bootstrapped Data Envelopment Analysis. In the second stage, the impacts of organizational and environmental variables on efficiency are investigated. Results show an increase of the efficiency gap between the best practice and the regions with repayment plan. It seems that reallocation of resources could increase the relative quality among hospitals in our sample. Further, higher quality in some dimensions of care need not be achieved as a result of higher costs or through reduced access to health care. It is considered essential to leverage the unexploited potential of E-Health in Italy.

Negli ultimi anni l’interesse per le misurazioni di efficienza in ambito sanitario è cresciuto sensibilmente. Molto spesso le misure di efficienza si concentrano maggiormente sulla parte produttiva tralasciando l’aspetto qualitativo poichè di difficile misurazione. L’obiettivo di questo lavoro è quello di sviluppare un modello per la valutazione dell’efficienza sia tecnica che qualitativa di una struttura sanitaria e capire se la presenza di differenze di tipo gestionale, informatico, finanziario, organizzativo, influenzino il valore di efficienza. Il modello è stato costruito e perfezionato basandosi sulla letteratura presente su queste tematiche e sull’esperienza empirica. Una Survey finale mostrerà lo stato di implementazione di alcune delle linee guida del Patto per la Salute 2014 – 2016 per verificare se alcune delle leve proposte dal questionario possano influenzare positivamente l’efficienza. L’analisi viene effettuata in due stadi. Nel primo viene misurata l’efficienza via bootstrap DEA, nel secondo stadio alcune variabili vengono regredite per conoscerne l’impatto sull’efficienza. I risultati dello studio mostrano un aumento del gap di efficienza tra le “best practice” e le regioni sottoposte a piano di rientro. Data le crescenti difficoltà economiche in cui versano queste regioni, si ritiene importante far leva sulle potenzialità ancora non sfruttate dell’ E-Health.
