This PhD Thesis presents the development of a biomimetic lower-limb neuroprosthesis for stroke patients, designed to facilitate motor relearning during gait training. Two inertial sensors placed on the shanks were used to detect in real time six sub-phases of the gait cycle. A controller mapped the stimulation strategy to the duration of the different gait sub-phases, so as to produce muscle activations synchronized with the patients’ current biomechanical task. The stimulation strategy was personalized to the patients' impairment, assessed through an initial study based on muscle synergies. A pilot randomized controlled trial tested the power of the system as a neurorehabilitation treatment. Four chronic stroke patients underwent an intensive treadmill training, with or without the stimulation. Preliminary results from these patients are presented and discussed.
La presente tesi di dottorato è focalizzata sullo sviluppo di una neuroprotesi biomimetica dell’arto inferiore per facilitare il riapprendimento motorio durante l’allenamento del cammino di pazienti colpiti da ictus. Due sensori inerziali, posizionati sulle tibie, sono stati usati per rilevare, in tempo reale, le sei fasi del ciclo del cammino. Un controllore volto all’individuazione della strategia di stimolazione muscolare rispetto alla durata delle diverse sotto-fasi del cammino, è stato sviluppato al fine di produrre attivazioni muscolari sincronizzate con l’attuale compito biomeccanico dei pazienti. La strategia di stimolazione è stata personalizzata in base al livello di compromissione dei pazienti, valutato attraverso uno studio iniziale basato su sinergie muscolari. Uno studio randomizzato controllato è attualmente in corso al fine di valutare l’efficacia della neuroprotesi come trattamento neuroriabilitativo. Nella presente tesi sono mostrati e discussi i risultati preliminari relativi a quattro pazienti affetti da ictus cronico, sottoposti a un allenamento intensivo su tapis-roulant, con e senza stimolazione.
