A structured approach to support the evolution of healthcare organizations in a value-based model

The increasing patients’ awareness of their health status, the continuous aging of the population and the diffusion of the chronic diseases are causing a growing demand of healthcare services and of their costs. This research is aimed at studying the evolution of the healthcare systems in order to meet these emerging needs of the population. From an organizational point of view, the healthcare systems have to improve their services, in order to offer an integrated care model that includes different services to be provided also at home. From a technological point of view, the main challenge is to introduce in the care processes new technologies in spite of the cultural and regulatory boundaries. The literature analysis underlines that the healthcare systems need a strategic approach that defines and guides their evolution towards a model that ensure the patient’s value, through a high quality of care services and low costs. For this reason, the analysis focuses on the “Value Based Health Care – VBHC” model, designed by Porter. This framework defines a strategy, based on six principles, to guide the healthcare organizations evolution towards a value-based model. The analysis focuses also on the application of the Porter’s VBHC model and it underlines that most of this real cases do not succeed because there is a lack of any guideline on how to implement the framework. For this reason, the goal of this thesis is to design a structured approach that helps the healthcare organizations to analyse their actual status and define the activities to perform in order to move towards a value-based model. In order to validate the designed structured approach, a European innovation project aimed at innovating the care processes and the related technological tools for patients affected by cognitive impairment was considered. In particular, the designed structured approach was used to map an Italian and a Swedish healthcare organization before and after the innovative solution introduction. The application of the structured approach to these real cases allows me to validate it and to underline the main weaknesses of the approach that I use to define the possible evolutions of the research.

La crescente consapevolezza dei cittadini sul tema salute, l’invecchiamento demografico e l’aumento delle patologie croniche fanno sì che la domanda di servizi legati alla Salute sia in ascesa e con essa anche i costi associati ai Sistema Sanitari. La ricerca ha dunque l’obiettivo di indagare come i Sistemi Sanitari stiano evolvendo al fine di rispondere alle sfide emergenti. A livello organizzativo, diversi Sistemi Sanitari stanno evolvendo verso una integrazione dei servizi sanitari e socio-sanitari offerti al paziente anche al domicilio. A livello tecnologico, la sfida è legata alla capacità dei Sistemi Sanitari di supportare adeguatamente il processo di adozione di nuove tecnologie, evolvendo in un settore caratterizzato da vincoli normativi e culturali. L’analisi di letteratura evidenzia la necessità di un approccio strategico che consenta di definire e guidare l’evoluzione verso un modello orientato al valore, che massimizzi il rapporto tra l’esito delle cure e la spesa sostenuta per generare quel risultato. La ricerca si focalizza dunque sull’analisi del model “Value Based Health Care – VBHC”, sviluppato da Porter, che ha l’obiettivo di definire una strategia, basata su sei principi, per guidare l’evoluzione delle organizzazioni sanitarie verso un modello orientato al valore. L’indagine condotta sull’applicazione delle organizzazioni del modello di Porter consente di evidenziare come ad oggi in molti casi le applicazioni pratiche siano caratterizzate da una comprensione superficiale dei principi. L’obiettivo del presente lavoro è quindi quello di disegnare un approccio strutturato per consentire alle organizzazioni sanitarie di analizzare il proprio stato e identificare i passi evolutivi verso un modello orientato al valore. Per la validazione dei risultati, si considerano due casi reali, svedese e italiano, di un progetto di ricerca europeo che ha l’obiettivo di innovare i processi di cura e le tecnologie a supporto di pazienti affetti da problemi cognitivi. Ciascun caso è mappato prima e dopo l’implementazione della soluzione innovativa tramite l’approccio proposto ed è così possibile validare il risultato della tesi ed individuarne gli sviluppi futuri.
