This paper deals with the topic of dust explosions and has as its purpose the drafting of a recursive operability analysis plus, applied to a real case, with the construction of the related fault tree. The concern of this project was the serious incident happened at the Zhongrong Metal Product CO in Kunshan, China, on 2 August 2014 which has involved more than a hundred people, including deceased and injured. The first chapter introduces the phenomenon of explosions in general, listing the main characteristics, the necessary conditions for this phenomenon to occur, the causes that can trigger them and the main consequences. Chapter 2 focuses on the powders, and describes the main characteristics that can lead to an explosion of this type, both primary and secondary, and the domino effects, and then focus on a specific group, ie metal powders. Then the thesis proceeds, in Chapter 3, with a list of measures to prevent and mitigate the risk of this type of event, explaining what aspects to focus on and the maneuvers to be carried out to prevent this phenomenon. Chapter 4 moves towards the main content of the project, as we list the main techniques used to carry out a risk analysis; among these the methodology of the ROA+ is deepened, fundamental for the analysis of the incidental event of interest. It also describes the methodology aimed at obtaining an estimate of the frequency of occurrence of an event, through the fault tree. Chapter 5 is dedicated to the incident previously mentioned, which took place in the ZRMP CO, in which car wheel polishing operations were carried out and where metal powders were present. In addition to the description of the plant and of the incidental dynamics, the extent of the damage recorded is listed and the possible sources of ignition responsible for the event are analyzed. We this work keeps on with Chapter 6, which contains the drafting of the ROA+, the description of the basic events and the search for the related Failure Rate values, which allow the reconstruction of the incident and, through the creation of the fault tree with the software OpenFTA, the probability of occurrence of the explosion is estimated. Finally, in Chapter 7, the conclusions of the paper are discussed, hypothesizing the improvement and above all preventive actions that could have prevented the occurrence of this catastrophe.
Il presente elaborato tratta il tema delle esplosioni da polveri e ha come scopo la stesura di un’analisi di operabilità ricorsiva plus applicata a un caso reale, con conseguente costruzione del relativo albero dei guasti. La motivazione di questo progetto è stato il grave incidente che ha interessato la Zhongrong Metal Product CO di Kunshan, Cina, il 2 agosto 2014, e che ha coinvolto più di un centinaio di persone, tra deceduti e feriti. Nel primo capitolo si introduce il fenomeno delle esplosioni in generale, elencandone le caratteristiche principali, le condizioni necessarie affinché si verifichi tale fenomeno, le cause che possono scatenarle e le principali conseguenze. Il Capitolo 2 pone l’attenzione sulle polveri, e descrive le principali caratteristiche che possono comportare un’esplosione di questo tipo, sia primaria che secondaria, e gli effetti domino, per poi concentrarsi su un gruppo specifico, ovvero le polveri metalliche. Si procede poi, nel Capitolo 3, con l’elenco delle misure di prevenzione e mitigazione del rischio di questo tipo di avvenimento, spiegando quali siano gli aspetti su cui porre l’attenzione e le manovre da realizzare per prevenire tale fenomeno. Con il Capitolo 4 ci si avvia verso il contenuto principale dell’elaborato, in quanto si elencano le principali tecniche utilizzate per svolgere un’analisi di rischio; tra queste si approfondisce la metodologia della AOR+, fondamentale per l’analisi dell’evento incidentale di interesse. Si descrive inoltre la metodologia volta ad ottenere una stima della frequenza di accadimento di un evento, attraverso l’albero dei guasti. Il Capitolo 5 è dedicato all’incidente precedentemente citato, che ha avuto luogo nella ZRMP CO, nella quale si svolgevano operazioni di lucidatura di cerchi per automobili e in cui erano presenti polveri metalliche. Oltre alla descrizione dell’impianto e della dinamica incidentale, si elenca l’entità dei danni registrati e si analizzano le possibili sorgenti di innesco responsabili dell’evento. Si giunge quindi al Capitolo 6, che contiene la stesura dell’AOR+, la descrizione degli eventi base e la ricerca dei relativi valori di Failure Rate, che consentono la ricostruzione dell’incidente e, attraverso la realizzazione dell’albero dei guasti con il software OpenFTA, si stima la probabilità di accadimento dell’esplosione avvenuta. Nel Capitolo 7, infine, si ragiona sulle conclusioni dell’elaborato, ipotizzando delle azioni migliorative e soprattutto preventive che avrebbero potuto evitare il verificarsi di tale catastrofe.
