In recent years there has been an increase in spending on healthcare that has not been matched by an increase in quality of the care provided. In this context Porter and Teisberg have advanced a new theoretical approach: Value-Based Healthcare (VBHC) which bases competition in healthcare on the value created for patients. VBHC is a theoretical framework that only recently has been implemented on real cases. The aim of this work is to implement VBHC approach in the preventive health sector. Starting from an innovative new machine for breast cancer screening the work explores possible service parameters to create a new preventive care service. Women need a service that helps them and guides them through the amount of misinformation and confusion that exist in prevention. The service must be time efficient and convey both the professionalism and experience needed during diagnostic testing and the calm and positive attitude that many women feel should be provided when doing prevention. The lack of a trustworthy source of information is thought to be one of the biggest hinders to women feeling secure and cared to by the healthcare system. At the end an idea of service is proposed: prevention services will be provided all on the same day in a dedicated pavilion of the hospital, with spa treatments that can be requested between examinations. There are also three ways of paying, the two traditional ones: through the national healthcare service and out of pocket/private insurance and a more innovative one by exchanging lifestyle data (freemium model).
Negli ultimi anni la spesa nel campo della sanità è aumentata, eppure questo aumento non è stato seguito da un eguale incremento nella qualità delle cure. In questo contesto due studiosi Porter e Tesiberg hanno proposto un nuovo modello: assistenza sanitaria basata sul valore ossia ritorno per il paziente (Value-Based Healthcare VBHC). Questo modello incentiva la competizione in sanità basata sul valore creato per il paziente. Questo modello è chiaro nella sua teoria ma ha ancora pochi sviluppi nella pratica. Partendo da una nuova tecnologia per la diagnosi precoce del tumore al seno, questa tesi si propone di identificare importanti parametri per disegnare un nuovo servizio di prevenzione utilizzando un approccio VBHC. Le donne necessitano di un sistema che riduca i tempi morti e che creai un’atmosfera di tranquillità e serenità senza ridurre la professionalità e la serietà della prevenzione. La mancanza di una figura competente e di fiducia a cui rivolgersi è uno dei maggiori problemi che impedisce di fare prevenzione con serenità e sentendosi ben assistite dal sistema. Alla fine, un’idea di un possibile servizio è proposta: tutti i servizi di prevenzione sono erogati lo stesso giorno uno dopo l’altro in un padiglione dedicato dell’ospedale ed è possibile richiedere servizi di spa tra un esame e il successivo. Inoltre, si propongono tre metodi di pagamento i due tradizionali: attraverso il servizio sanitario nazionale, con un’assicurazione privata e uno più innovativo basato sullo scambio di dati di lifestyle (modello freemium)
