Creating a Value Based Healthcare, that is an healthcare based on the health outcomes generated rather than on the volumes of services provided, is the most important challenge that awaits us in order to maintain our universal and sustainable healthcare system, to respond to the care needs of the population, avoiding further increases in costs and guaranteeing quality and innovation. The active and conscious participation of citizens in health is an issue that is becoming increasingly important in this landscape and can generate an impact and social, clinical and economic value, a real added value for health. The factors that are making health care in difficulty today are the ageing of the population and the consequent increase in the incidence of chronic diseases. Hospitals are dedicated to acute care but are not equipped for long-term patient care. Currently, it is possible to develop digital technologies capable of reading the genome of an individual and giving birth to the so-called precision medicine, i.e. the attempt to customize as much as possible diagnosis, treatment and, first of all, prevention. It represents one of the most advanced methodologies in this field, recognizing each patient's uniqueness for a multidisciplinary and patient-centric approach to care. However, this approach is prohibitive in terms of costs and skills and therefore still difficult to implement for everyone. The answer to these needs can consist in the introduction of an innovative approach that starts from the education of the citizen/patient in order to generate empowerment and make each one proactive and protagonist of his own care path. In this work, the response of the Centro Cardiologico Monzino, I.R.C.C.S. of Milan, the popular event “Milano Heart Week”, was analysed. It is a series of events on the occasion of the World Heart Day to raise awareness of the importance of prevention. The aim is the creation of an empowered community towards a life of quality and greater sustainability of care and expenditure of the Health Service in a win-win logic. The study starts from the identification of homogeneous groups of individuals on which to set up appropriate models of health communication, based on socio-demographic characteristics and lifestyles because the customization of the treatment comes from the setting up of a tailor-made prevention and therefore from a customization of communication. Since changing one's lifestyle is difficult, it is necessary to act quickly on an individual before he begins to take on an incorrect one. Therefore, education for prevention on young people becomes fundamental for the achievement of the objectives previously set out. The paper therefore contains an in-depth examination of this target group. Following the analysis of the outputs of the event in terms of individuals reached, the long-term results are studied, and so the outcomes, for the verification of the effectiveness and efficiency of the event through the collection of feedback from participants. These evaluations are developed, finally, in analysis resulting in the realization of significant proposals and management implications.
Creare una sanità basata sul valore, ovvero fondata sugli esiti di salute generati anziché sui volumi di prestazioni erogate, è la sfida più importante che ci attende per mantenere il nostro sistema così universalistico e sostenibile, per rispondere ai bisogni assistenziali della popolazione, evitando di aumentare i costi e garantendo qualità e innovazione. La partecipazione attiva e consapevole dei cittadini nella salute è un tema che sta acquistando sempre più rilevanza in questo panorama e può generare un impatto e un valore sociale, clinico ed economico, un reale valore aggiunto per la sanità. La difficoltà dell’assistenza sanitaria oggi dipende dall’invecchiamento della popolazione e il conseguente aumento dell’incidenza delle malattie croniche. Gli ospedali accolgono episodi acuti ma non sono attrezzati per una presa in carico del paziente a lungo termine. Attualmente è stato possibile realizzare tecnologie digitali in grado di leggere il genoma di un individuo e partorire la cosiddetta medicina di precisione, ovvero il tentativo di personalizzare il più possibile diagnosi, cure e, per prima, la prevenzione. Essa è una delle metodologie più all’avanguardia in questo ambito, riconoscendo a ogni paziente la propria unicità per un approccio, multidisciplinare e paziente-centrico alle cure. Questo approccio è però proibitivo in termini di costi e competenze e quindi ancora difficile da implementare per tutti. La risposta a queste esigenze può consistere nell'introduzione di un approccio innovativo che parta dall’educazione del cittadino/paziente al fine di generare empowerment e rendere ciascuno proattivo e protagonista del proprio percorso di cura. In questo lavoro è stata analizzata la risposta del Centro Cardiologico Monzino, I.R.C.C.S. di Milano che ha realizzato un evento divulgativo, la Milano Heart Week. Si tratta di una serie di eventi in occasione della giornata mondiale per il cuore per sensibilizzare l’individuo sull’importanza della prevenzione e per la creazione di una community potenziata verso una vita di qualità e una maggiore sostenibilità delle cure e delle spese del Servizio Sanitario in una logica win-win. Lo studio parte dall’ identificazione di gruppi omogenei di individui sui quali impostare modelli di comunicazione sanitaria adeguati, basati sulle caratteristiche sociodemografiche e gli stili di vita perché la personalizzazione della cura nasce dall’impostazione di una prevenzione su misura e quindi da una personalizzazione della comunicazione. Poiché cambiare il proprio stile di vita è difficile bisogna agire presto su un individuo prima che inizi ad assumerne uno scorretto. Ecco che l’educazione alla prevenzione sui giovani diventa fondamentale per il raggiungimento degli obiettivi precedentemente esposti. L’elaborato contiene quindi un approfondimento su questo target. A seguito dell’analisi degli output dell’evento in termini di individui raggiunti, vengono studiati i risultati di lungo termine, ovvero gli outcomes, per la verifica dell’efficacia e dell’efficienza dell’evento mediante la raccolta di feedback da parte dei partecipanti. Queste valutazioni si sviluppano, infine, in analisi sfocianti nella realizzazione di significative proposte e implicazioni gestionali.
