The health emergency related to the worldwide spread of Covid-19, which characterized the whole 2020, effectively stressed the Italian National Health System. It is clear how the health structures of the Lombardy region have been affected, especially during the months of March and April. In order to try to contain the infection in view of a new wave, that’s very important to provide a careful hospital planning, which is developed from the analysis of the difficulties, occurred in the course of the first emergency period. The thesis research work has been carried out within this shown context and involved the analysis of the impact experienced by the Azienda Socio-Sanitaria Territoriale (ASST) Spedali Civili of Brescia, one of the most affected by the virus, and the analysis of the management plans, that consequently the Healthcare Agency itself had to implement to try to minimize its effects. The analysis conducted focuses on describing what happened both in the first period of spread of the virus during about the spring months, and in the post-emergency period during the summer months, paying attention to the structural changes that have come about inevitably. In particular, great attention has been paid to the project called ‘scala 4’, which provides an example of the way in which today a new hospital setting-up takes place, built in an area of the healthcare company previously little exploited. The project consists in the construction of a healthcare center dedicated only to positive Covid patients, representing a great innovation in the hospital care offering scenario since it is the hospital that has to adapt to the patient's needs and not the opposite. This thesis, also, aims to describe the variations that the Clinical Engineering Service has undergone in the same time periods, focusing on the operations concerning the installation, testing, corrective and extraordinary maintenance, as they have been mostly followed during the months of the internship. The tools, used to perform the studies described, were company documents and software, university seminars, journalistic reports regarding the healthcare sector, as well as interviews with ASST employees. The work that has been carried out allows to underline organizational problems inside the processes of Italian hospitals, which in the future will have to be attenuated and solved in some way.
L’emergenza sanitaria legata alla diffusione in tutto del mondo del Covid-19, che ha caratterizzato interamente l’anno 2020, ha testato efficacemente la solidità del Sistema Sanitario Nazionale Italiano. È noto a tutti quanto la sanità della regione Lombardia ne abbia risentito, in particolar modo nei mesi di marzo e aprile. Fondamentale per provare a contenere il contagio in vista di una nuova ondata è un’attenta programmazione ospedaliera, che parte dall’analisi delle difficoltà che gli ospedali stessi hanno avuto nel primo periodo emergenziale. È in questo contesto che si inserisce il seguente lavoro di ricerca, dedicato all’analisi dell’impatto subito dall’Azienda Socio-Sanitaria Territoriale (ASST) Spedali Civili di Brescia, una tra le maggiormente colpite dal virus, e all’analisi dei piani di gestione che conseguentemente l’azienda stessa ha dovuto attuare per provare a minimizzarne gli effetti. L’analisi condotta si concentra a descrivere quanto è avvenuto sia durante il primo periodo di diffusione del virus coincidente indicativamente con i mesi primaverili, sia durante il periodo post emergenziale coincidente con i mesi estivi, ponendo l’attenzione ai cambiamenti strutturali che si sono resi necessari. In particolar modo, grande attenzione è posta al progetto di scala 4, il quale fornisce un esempio della modalità attraverso la quale al giorno d’oggi avviene l’allestimento in da principio di un nuovo reparto ospedaliero, costruito in un’area dell’azienda precedentemente poco sfruttata. Il progetto consiste nella realizzazione di un centro di cura dedicato ai soli pazienti Covid positivi, rappresentando una grande innovazione nello scenario di offerta di assistenza ospedaliera dal momento che è l’ospedale a doversi adeguare ai bisogni del paziente e non viceversa. La presente tesi mira anche a descrivere le variazioni che negli stessi periodi temporali l’Unità Operativa Complessa Ingegneria Clinica ha subito, focalizzandosi su quel che riguarda le procedure di ‘Installazione e Collaudi’ e ‘Manutenzioni Correttive e Straordinarie’, in quanto maggiormente seguite durante i mesi dello stage. Gli strumenti utilizzati per eseguire gli studi descritti sono stati documenti e software aziendali, seminari universitari, report giornalistici riguardanti l’ambito sanitario, oltre a interviste ai dipendenti dell’ASST. Il lavoro svolto permette di far emergere chiaramente delle problematiche organizzative all’interno dei processi delle aziende ospedaliere italiane, che in futuro in qualche maniera bisognerà tentare di arginare e risolvere.
