Failure analysis of a pressure vessel catastrophic collapse: technical causes and legal effects

The scope of this work is the investigation of a real case of high-pressure vessel’s collapse by using the Failure Analysis approach. The case had regrettably dramatic consequences for an inspector during the performance of the hydraulic test. A thorough investigation on the solutions adopted during the design as well as during the manufacturing and testing phases are reported pointing out the nonconformances with respect to the norms and proposing alternative solutions. The whole investigation is not only a technical analysis, but it takes also into consideration the legal aspects involved.

Questa tesi ha come obiettivo quello di investigare nella logica della Failure Analysis un reale caso di collasso di un serbatoio ad alta pressione, il quale purtroppo ebbe delle conseguenze drammatiche per un operatore durante l’effettuazione delle prove di collaudo statico. Nel corso della trattazione vengono analizzate le scelte effettuate durante la fase progettuale, quella realizzativa e di collaudo, evidenziando le non conformità rilevate e proponendo soluzioni alternative. L’indagine non si limita ad effettuare un’analisi tecnica ma esamina anche aspetti di carattere giuridico-legale.
