Myocarditis is a challenging and complex inflammatory disease of the myocardium, usually secondary to viral infections or immune dysregulating phenomena. Inflammation can lead to life-threatening ventricular arrhythmias, through unknown pathological mechanisms, both in the acute and chronic stage. A suspected pathway of myocarditis-induced arrhythmias is the damaging of cardiac tissue after the immune response, which leads to the formation of scar tissue. The latter impairs the physiological electrical activity of the heart, causing arrhythmias, which can be detected and traced back to the inflammatory state. Current experimental models for cardiac electrophysiology range from subcellular structures to the individual patients. Vital cardiac slices combine both the structural complexity and the ease of data interpretation. At the same time, their thinness allows the diffusion of oxygen and other metabolic substrates into the inner cells, preserving viability ex vivo. In this work, murine ventricular slices have been used to design an experimental protocol for the development of a pathological model for arrhythmic myocarditis. Healthy ventricular slices were studied on micro-electrode arrays to detect cardiac signals and optimize all the set-up parameters. Continuous perfusion with Tyrode’s solution and oxygenation were provided to preserve viability of the tissue by means of a custom-designed circuit. Electrophysiological activity was subsequently detected and analyzed using MC_Rack software. The experimental outcomes are promising and foster further study.
La miocardite è una malattia infiammatoria complessa del miocardio, solitamente provocata da infezioni virali o fenomeni di natura autoimmune. L'infiammazione, sia in fase acuta che cronica, può causare aritmie ventricolari letali attraverso meccanismi patologici ancora sconosciuti. Una delle ipotesi per le cause di aritmia è la formazione di tessuto cicatriziale fibrotico in seguito al danneggiamento del tessuto cardiaco dovuto alla risposta immunitaria. La cicatrice compromette l'attività elettrica fisiologica del cuore, causando aritmie, che possono essere rilevate e, successivamente, ricondotte allo stato infiammatorio. Attualmente, gli studi di elettrofisiologia cardiaca includono una grande varietà di modelli, dalle strutture subcellulari ai singoli pazienti. Le fette cardiache vitali combinano la complessità strutturale e la facilità di interpretazione dei dati, contemporaneamente il loro spessore ridotto permette la diffusione di ossigeno e altri substrati metabolici negli strati cellulari più interni, preservando la vitalità ex vivo. In questo lavoro, le fette ventricolari murine sono state utilizzate per progettare un protocollo sperimentale per lo sviluppo di un modello patologico di miocardite aritmica. Le fette ventricolari sane sono state studiate su micro-array di elettrodi per rilevare i segnali cardiaci e ottimizzare tutti i parametri della configurazione. Per preservare la vitalità del tessuto è stato realizzato un circuito ad hoc per garantire la perfusione continua con la soluzione di Tyrode e l'ossigenazione. L'attività elettrofisiologica è stata successivamente rilevata e analizzata utilizzando il software MC_Rack. I risultati sperimentali sono promettenti e suggeriscono ulteriori studi.
