Stent-grafts are endovascular prosthesis based on the coverage of a stent through a polymeric membrane that demonstrated effective outcomes in the treatment of peripheral artery disease compared with bare metal stents, thanks to their ability to reduce the incidence of in-stent restenosis. Nevertheless, poor endothelialization capacity of the membrane materials currently used still represents a limitation. Silkothane®, a material obtained by blending of silk fibroin and polyurethane following the in situ tissue engineering approach, represents a promising solution and was then used in this work to cover a stent internally and externally through electrospinning. This innovative stent-graft was manufactured in two configurations, which were morphologically and mechanically characterized, through a combined in vitro-in silico approach, to evaluate their behavior during the crimping and expansion process.
Gli stent-grafts sono protesi endovascolari che consistono nella ricopertura di uno stent con una membrana polimerica. Questi hanno dimostrato buoni risultati clinici nel trattamento dell'arteriopatia periferica rispetto ai bare metal stents, grazie alla loro capacità di ridurre il fenomeno della restenosi intra-stent. Tuttavia, i materiali usati oggi per la membrana non promuovono il processo di endotelizzazione e questo rappresenta ancora una limitazione nell'utilizzo di questi dispostivi. Il Silkothane®, un materiale ottenuto dalla miscela della fibroina della seta e del poliuretano che si inserisce nell'ambito dell'ingegneria tissutale in situ, rappresenta una potenziale soluzione a questo problema ed è quindi stato utilizzato in questo lavoro per ricoprire uno stent internamente ed esternamente tramite elettrospinning. Questo stent-graft innovativo è stato prodotto in due diverse configurazioni, che sono state caratterizzate parallelamente in vitro e in silico dal punto di vista morfologico e meccanico, per valutare il loro comportamento durante il processo di crimping e di espansione.
