Towards proximity healthcare : PPP as a model for the long-term management of case di comunità: economic, social and environmental benefits

The aim of this thesis is to propose a model for the long-term management of "Case di Comunità". They are public structures that aim to promote local health care, and are among the strategies proposed by the PNRR, in particular in Mission 6 on Health. An investment of € 2 billion is planned for the activation of 1288 Case di Comunità until 2026, year after which the structures should proceed independently from the financial point of view. Within the framework of the Italian Sistema Sanitario Nazionale (SSN), where there has been a long-standing shortage of health workers and a general reduction in the resources allocated to the sector, these structures cannot be managed exclusively by the public sector, but the need for collaboration with private entities is expected. This work of thesis aims to propose the Public Private Partnership (PPP) as more effective approach in terms of expected returns and risk management, because the private subject, not only engages from the economic point of view, but also in the management of the structure. For a broader view on the subject, some reference models and some national and international cases were analysed and were used as a reference to develop the PPP model proposed specifically for the case of Case di Comunità. Economic, social and environmental benefits deriving from the adoption of this approach are highlighted.

L' obiettivo della tesi è proporre un modello per la gestione a lungo termine delle "Case di Comunità". Si tratta di strutture pubbliche che mirano a promuovere l'assistenza sanitaria locale, e sono tra le strategie proposte dal PNRR, in particolare nella missione 6 sulla salute. Un investimento di 2 miliardi di euro è previsto per l'attivazione di 1288 Case di Comunità fino al 2026, anno dopo il quale le strutture dovrebbero procedere autonomamente dal punto di vista finanziario. Nell'ambito del Sistema Sanitario Nazionale Italiano (SSN), dove da tempo si registra una carenza di operatori sanitari e una generale riduzione delle risorse destinate al settore, tali strutture non possono essere gestite esclusivamente dal settore pubblico, ma risulta necessaria una collaborazione con soggetti privati. Questo lavoro di tesi mira a proporre il Partenariato Pubblico Privato (PPP) come approccio più efficace in termini di rendimenti attesi e gestione del rischio, perché il soggetto privato, non solo si impegna dal punto di vista economico, ma anche nella gestione della struttura. Per una visione più ampia dell'argomento, sono stati analizzati alcuni modelli di riferimento e alcuni casi nazionali e internazionali che sono stati utilizzati come riferimento per sviluppare il modello di PPP proposto specificamente per il caso di Case di Comunità. Sono evidenziati i benefici economici, sociali e ambientali derivanti dall'adozione di questo approccio.
