Introduction: As the global population ages, the prevalence of neurodegenerative diseases, particularly dementia, is increasing. Presently, there are no cures for dementia, only preventive measures. Mild Cognitive Impairment (MCI), identified as a pre-dementia stage between normal and confirmed dementia, poses diagnostic challenges. However, early detection of MCI holds potential in preventing the progression to dementia. Therefore, early, and vigilant monitoring of patients to detect MCI conditions is crucial. Methods: for this study, 55 subjects with MCI were recruited, and data collection utilized a sensor-equipped pen. Subjects underwent three tasks: writing a grocery list, composing free text, and dictating 15 fixed words (PnP). The collected data allowed the extraction of writing indicators, upon which the following analyses were conducted: test-retest reliability and validity analysis of the indicators, and differences analysis between word types. Results: In the grocery list and free text tasks, 89.8% of cursive allograph indicators were reliable, contrasting with a lower 61.2% reliability for block letters allograph. For the PnP task, cursive allograph had 60% reliability, while block letters allograph had 64%. The study noted the highest correlations in clinical tests MMSE and IADL. Generally, cognitively intact individuals showed faster task performance, fewer hesitations, and fewer pauses. Analyzing different word types, temporal indicators indicated increased difficulty in writing irregular words. Conclusions: Excellent results were obtained, supporting the use of the tool for monitoring MCI subjects in both controlled and uncontrolled tests.
Introduzione: Con l'invecchiamento della popolazione globale, la prevalenza delle malattie neurodegenerative, in particolare la demenza, sta aumentando. Attualmente non esistono cure per la demenza, solo misure preventive. L'Impairment Cognitivo Lieve (ICL), identificato come una fase pre-demenza tra la normale funzionalità e la demenza confermata, pone sfide diagnostiche. Tuttavia, il rilevamento precoce dell'ICL ha il potenziale per prevenire la progressione verso la demenza. Pertanto, il monitoraggio precoce e attento dei pazienti per rilevare le condizioni di ICL è cruciale. Metodi: Per questo studio, sono stati reclutati 55 soggetti con ICL e la raccolta dati ha utilizzato una penna dotata di sensori. I soggetti hanno svolto tre compiti: scrivere una lista della spesa, comporre testo libero e dettare 15 parole fisse (PnP). I dati raccolti hanno permesso l'estrazione di indicatori di scrittura, sui quali sono stati condotti i seguenti analisi: affidabilità test-retest e analisi di validità degli indicatori, e analisi delle differenze tra tipi di parole. Risultati: Nei compiti di lista della spesa e testo libero, l'89,8% degli indicatori di allografi corsivi è risultato affidabile, a differenza di una affidabilità inferiore del 61,2% per gli allografi in stampatello. Per il compito PnP, l'allografo corsivo ha avuto una affidabilità del 60%, mentre quello in stampatello ha avuto il 64%. Lo studio ha notato le correlazioni più elevate nei test clinici MMSE e IADL. In generale, gli individui cognitivamente integri hanno mostrato una performance più veloce nei compiti, minori esitazioni e meno pause. Analizzando diversi tipi di parole, gli indicatori temporali hanno indicato una maggiore difficoltà nella scrittura di parole irregolari. Conclusioni: Sono stati ottenuti ottimi risultati, supportando l'uso dello strumento per il monitoraggio dei soggetti con ICL sia in test controllati che non controllati.
