Failure Analysis of a liquefied petroleum gas (LPG) storage vessel

An event is defined accident when a fatality occurs: the Failure Analysis studies the accidents to learn and define what was wrong leading to that failures and to prevent its occurence. The aim of this thesis is to investigate the causes and the subsequent failure phenomenon that led to the fatal collapse of a LPG storage vessel occurred in July 2019 at Gravedona ed Uniti (CO) during the first fullfilling. Consequently, a penal proceeding has been carried to clarify the events that caused the mortal accident, outlining responsibilities and faults. In this perspective, a Failure Analysis activity will be conducted, and the role of the engineer as a technical consultant during a criminal litigation will be treated. Throughout the discussion, the results of the tests conducted on the wreckage of the vessel will be analyzed in order to discuss the actions and events that led to the collapse. Additionally, corrective actions that can contribute to preventing future failures of this kind will be further examined.

Un evento è definito incidente quando vi è una fatalità: la Failure Analysis studia gli incidenti al fine di apprendere e definire cosa ha concorso a tale failure e per prevenirne la ricorrenza. L'obiettivo di questa tesi è quello di investigare le cause e il conseguente fenomeno di failure che hanno portato al fatale collasso di un serbatoio per lo stoccaggio di GPL avvenuto a luglio 2019 presso la località di Gravedona ed Uniti (CO) durante la fase di primo riempimento. Conseguentemente un procedimento penale è stato istituito al fine di chiarire gli eventi che portarono all'incidente mortale delineando responsabilità e colpe. In quest'ottica un'attività di Failure Analysis verrà condotta e verrà trattato il ruolo dell'ingegnere come consulente tecnico durante un contenzioso di tipo penale. Nel corso della trattazione verranno analizzati i risultati dei test eseguiti sui relitti del serbatoio al fine di discutere le azioni ed gli eventi che hanno portato al collasso. Verranno profonditi inoltre le azioni correttive che possono concorrere a prevenire future failure di questo tipo.
