Digital health technologies are widely used to support patient care, self-management, and clinical decision making processes. However, their effectiveness also depends on user experience and accessibility, particularly for vulnerable populations. Although usability is frequently evaluated, accessibility and cognitive load are rarely assessed using objective methods. This thesis examines the use of eye-tracking for evaluating user experience and accessibility in digital health applications from a biomedical engineering perspective. A systematic scoping review was conducted in accordance with PRISMA-ScR guidelines. Sixteen empirical studies published between 2014 and 2025 were included and analyzed in terms of application domains, eye-tracking metrics, experimental setups, and the representation of vulnerable user groups. The results indicate substantial methodological heterogeneity across studies. Eye tracking is primarily employed for usability evaluation, while accessibility is rarely addressed explicitly. Vulnerable populations remain underrepresented outside assistive technology contexts, and gaze-based metrics are not systematically linked to accessibility outcomes or cognitive load interpretation. Based on these findings, the Eye-Tracking–Based Accessibility Evaluation Framework (ETAEF) is proposed. The framework integrates eye-tracking metrics, cognitive load indicators, and accessibility-related user experience dimensions. This work positions eye-tracking as a biomedical measurement tool that supports more objective and safety-oriented evaluation practices in digital health systems.
Le tecnologie di digital health sono ampiamente impiegate per supportare l’assistenza ai pazienti, l’autogestione e i processi decisionali clinici. Tuttavia, la loro efficacia dipende anche dall’esperienza utente e dall’accessibilità, soprattutto per le popolazioni vulnerabili. Sebbene l’usabilità sia frequentemente valutata, l’accessibilità e il carico cognitivo sono raramente analizzati mediante metodi oggettivi. Questa tesi esamina l’impiego dell’eye-tracking per la valutazione dell’esperienza utente e dell’accessibilità nelle applicazioni di digital health, adottando una prospettiva di ingegneria biomedica. È stata condotta una systematic scoping review secondo le linee guida PRISMA-ScR. Sono stati inclusi sedici studi empirici pubblicati tra il 2014 e il 2025, analizzati in relazione ai domini applicativi, alle metriche utilizzate, ai setup sperimentali e alla rappresentazione degli utenti vulnerabili. I risultati evidenziano un’elevata eterogeneità metodologica. L’eye-tracking è impiegato prevalentemente per valutazioni di usabilità, mentre l’accessibilità è raramente affrontata in modo esplicito. Le popolazioni vulnerabili risultano sottorappresentate al di fuori del contesto delle tecnologie assistive, e le metriche di sguardo non sono sistematicamente collegate agli esiti di accessibilità o al carico cognitivo. Sulla base di tali evidenze, viene proposto l’Eye-Tracking–Based Accessibility Evaluation Framework (ETAEF), che integra metriche di eye-tracking, indicatori di carico cognitivo e dimensioni dell’esperienza utente rilevanti per l’accessibilità. Il lavoro posiziona l’eye-tracking come strumento di misurazione biomedica a supporto di pratiche di valutazione più oggettive e orientate alla sicurezza nei sistemi di digital health.
