Breast cancer is often treated with a combination of surgery and radiotherapy (RT). Radiation exposure may cause cardiac toxicity when the heart is in the radiation field. Therefore, the prediction of the onset of possible cardiovascular diseases (CVDs) after radiotherapy is crucial to ensure good health conditions for breast cancer patients. The goal of this project is to investigate the predictive role of cardiac calcifications (CACs) associated with cardiac substructures, in order to determine whether information related to the spatial distribution of CACs within the heart may lead to improved prediction models compared to models based on CACs without including spatial information. The considered population consists of 994 breast cancer patients. The sample is characterized by a high class imbalance between patients who suffer from cardiac toxicity (positive class) and those who do not (negative class). To address this problem, several models are applied and compared: two bivariate logistic regression models with bootstrap and an ensemble classifier, which combines a weighted logistic regression with a class-weighted random forest. The results indicate that the calcifications of the left anterior descending artery (A_LAD) are better predictors of post-RT CVD events, with respect to whole-heart CAC scores. Lastly, it is observed that more complex models achieve better performance, despite the tendency to overfit due to the limited number of positive cases in the dataset.

Il tumore della mammella è spesso trattato con una combinazione di chirurgia e radioterapia (RT). L’esposizione alle radiazioni può provocare tossicità cardiaca quando il cuore è nel campo delle radiazioni. Pertanto, la predizione dell’insorgenza di possibili malattie cardiovascolari (CVDs) dopo la radioterapia è cruciale per garantire buone condizioni di salute ai pazienti affetti da tumore del seno. L’obiettivo di questo progetto è indagare il ruolo predittivo delle calcificazioni cardiache (CACs) associate a specifiche sottostrutture del cuore, per determinare se informazioni relative alla distribuzione spaziale delle CACs nel cuore possano portare a migliori modelli predittivi rispetto a modelli basati sulle CACs senza l’inclusione di informazioni spaziali. La popolazione considerata è costituita da 994 pazienti affetti da tumore della mammella. Il campione è caratterizzato da un forte sbilanciamento tra pazienti affetti da cardiotossicità (classe positiva) e non (classe negativa). Per affrontare tale problema, più modelli sono applicati e confrontati: due regressioni logistiche bivariate con bootstrap e un classificatore ensemble, che combina una regressione logistica pesata con una random forest con pesi per le classi. I risultati indicano che le calcificazioni dell’arteria discendente anteriore sinistra (A_LAD) sono predittori migliori di CVD post-RT, rispetto ai CAC scores relativi all’intero cuore. Infine, si osserva che modelli più complessi raggiungono migliori performance, nonostante la tendenza all’overfitting dovuta al numero limitato di casi positivi nel dataset.
