Constructing a large-scale hospital involves significant coordination between stakeholders, disciplines, and documentation frameworks. Effective management of documents is crucial to ensure that design intentions, technical specifications, and construction instructions can be communicated clearly and on time. Document breakdowns in the construction process often result in uncoordinated work, rework, delays, and budget overruns. This thesis examines request for information (RFI), and document control issues founded by two general contractors and the trade partners across three healthcare projects in the United States each over $300 million, to assess documentation bottlenecks and breakdowns. For this study, the RFI and documents that had been classified were coded and analyzed using a structed RFI classification and issue classification cross frameworks to assess the scope and magnitude of patterns, project phases, and gap variables for relative comparison. Annually, nearly 31 billion USD is spent in rework due to inaccurate or inaccessible information in construction (Trimble Inc., 2025). Addressing this issue begins with identifying the major causes of workflow information breakdowns during phases transition, or in critical coordination points such as mechanical-electrical-plumbing. Key categories of issues were identified, including missing information, version, and revision control, naming and index inconsistencies, among others. Subsequently, quantitative, and qualitative analysis were conducted, and temporal and cross-project comparisons showed consistent bottlenecks around milestones/revisions and model handover, highlighting the information management challenges despite the different general contractors and trades involve in each project. This study contributes empirical evidence on when and where documentation breakdowns are most frequent and give insights on how effective document control practices and knowledge management may mitigate these disruptions. Recommendations include standardization, implementing clear workflows of information es and early coordination protocols including metadata practices from the earliest stages of projects and real time validation in later phases.
Costruire un ospedale su larga scala comporta una significativa coordinazione tra le parti interessate, le discipline e i quadri documentali. Una gestione efficace dei documenti è cruciale per garantire che le intenzioni di design, le specifiche tecniche e le istruzioni di costruzione possano essere comunicate in modo chiaro e tempestivo. Le interruzioni documentali nel processo di costruzione spesso risultano in lavori non coordinati, rifacimenti, ritardi e sforamenti di budget. Questa tesi esamina le richieste di informazioni (RFI) e i problemi di controllo dei documenti riscontrati da due appaltatori generali e dai partner commerciali in tre progetti sanitari negli Stati Uniti, ciascuno superiore ai 300 milioni di dollari, per valutare i colli di bottiglia e i guasti nella documentazione. Questo studio contribuisce con prove empiriche su quando e dove i guasti nella documentazione sono più frequenti e fornisce indicazioni su come pratiche efficaci di controllo dei documenti e gestione della conoscenza possano mitigare queste interruzioni. Le raccomandazioni includono la standardizzazione, l'implementazione di flussi di lavoro chiari delle informazioni e protocolli di coordinamento precoce che includano pratiche di metadati fin dalle prime fasi dei progetti e la validazione in tempo reale nelle fasi successive.
