Progressive myopia has emerged as a major global health concern, projected to affect nearly 50% of the population by 2050. This condition is characterized by excessive axial elongation of the eyeball, driven by scleral thinning and biomechanical weakening. Unlike conventional optical or pharmacological treatments that manage visual signaling, this thesis investigates scleral collagen crosslinking (SXL) as a direct biomechanical intervention to reinforce the scleral extracellular matrix and improve its mechanical stability. The primary scientific objective of this work was the experimental characterization of scleral stiffness following a photo-oxidative crosslinking protocol. Utilizing porcine eyes as a representative ex vivo [AB1.1]model due to their morphological and histological similarities to human eyes, this study employed a riboflavin-UVA (370nm) protocol. A total of 76 scleral specimens were analyzed to evaluate the treatment’s efficacy in the posterior temporal (POST-TEMP) and posterior nasal (POST-NAS) regions. The methodology involved uniaxial tensile testing to quantify the tangent modulus (\operatorname{E}_t) across relevant strain levels ranging from 2% to 10%. The experimental results demonstrate a significant biomechanical reinforcement of the tissue. In the POST-TEMP region, treated specimens exhibited a statistically significant increase in the tangent modulus at strain levels of 4% and above, reaching a maximum average increase of 44.54% at 10% strain. Interestingly, the study revealed marked regional variations: while the temporal sclera showed robust stiffening, the POST-NAS region exhibited a lower, non-statistically significant increase in stiffness (maximum increase of 13.45% at 6% strain). These findings suggest that regional differences in scleral microstructure and collagen architecture influence SXL efficacy. The relevance of this research lies in providing a proof of concept for SXL as a mechanical reinforcement strategy for myopia control. By increasing the stability of the collagenous framework, the treatment enhances resistance to tensile deformation, potentially addressing one of the biomechanical mechanisms underlying scleral remodeling. This work establishes a foundation for future developments in minimally invasive delivery systems and long term in vivo validation, positioning SXL as a promising avenue for preventing the progression of pathological myopia.
La miopia progressiva è attualmente una delle principali problematiche di salute pubblica a livello globale, che si prevede colpirà il 50% della popolazione mondiale entro il 2050. Questa condizione è caratterizzata da un eccessivo allungamento assiale del bulbo oculare, determinato dall'assottigliamento della sclera e dal suo indebolimento biomeccanico. A differenza dei trattamenti ottici o farmacologici convenzionali, che agiscono principalmente sulla modulazione dei segnali visivi, la presente tesi indaga il crosslinking del collagene sclerale (SXL) come intervento biomeccanico diretto, volto a rinforzare la matrice extracellulare della sclera e migliorarne la stabilità meccanica. L'obiettivo principale di questo lavoro è stato caratterizzare sperimentalmente la rigidezza sclerale a seguito di un protocollo di crosslinking foto-ossidativo. Utilizzando occhi porcini come modello ex vivo rappresentativo, grazie alle loro somiglianze morfologiche e istologiche con l'occhio umano, lo studio ha impiegato un protocollo a base di riboflavina e raggi UVA (370 nm). Sono stati analizzati complessivamente 76 campioni sclerali per valutare l'efficacia del trattamento nelle regioni temporale posteriore (POST-TEMP) e nasale posteriore (POST-NAS). La metodologia ha previsto prove di trazione uniassiale per quantificare il modulo tangente (\operatorname{E}_t) a livelli di deformazione compresi tra il 2% e il 10%, considerati biomeccanicamente rilevanti. I risultati sperimentali dimostrano un significativo rinforzo biomeccanico del tessuto trattato. Nella regione POST-TEMP i campioni trattati hanno mostrato un incremento statisticamente significativo del modulo tangente per deformazioni pari o superiori al 4%, raggiungendo un incremento medio massimo del 44,54% a una deformazione del 10%. Lo studio ha inoltre evidenziato marcate differenze regionali: mentre la sclera temporale ha mostrato un consistente irrigidimento, la regione POST-NAS ha presentato un incremento della rigidezza inferiore e non statisticamente significativo con un massimo del 13,45% a una deformazione del 6%. Tali risultati suggeriscono che le differenze regionali nella microstruttura sclerale e nell’ organizzazione delle fibre di collagene influenzino l'efficacia del trattamento SXL. La rilevanza di questa ricerca risiede nel fornire una prova di concetto a supporto del trattamento SXL come strategia di rinforzo meccanico per il controllo della progressione della miopia. Incrementando la stabilità della struttura delle fibre di collagene, il trattamento aumenta la resistenza del tessuto alla deformazione, intervenendo potenzialmente su uno dei principali meccanismi di rimodellamento sclerale. Questo lavoro pone le basi per futuri sviluppi di sistemi di somministrazione mininvasivi e per la validazione in vivo a lungo termine, delineando il crosslinking sclerale come una strategia promettente per la prevenzione della miopia progressiva.
