Failure Mode and Effects Analysis (FMEA) is a widely used method for supporting risk analysis and safety decision-making in industrial systems, but its application is time-consuming, costly and strongly dependent on expert judgement. To address these challenges, this work proposes an agentic Artificial Intelligence (AI) framework for systematically generating and refining FMEA tables. Each field of the FMEA table is generated by a dedicated triplet of generator, evaluator and regenerator agents. The generator agent proposes candidate FMEA entries using domain and system-specific knowledge retrieved from a curated knowledge base through Retrieval Augmented Generation (RAG). The evaluator agent assesses the generated entries against the available domain and system knowledge and classifies them as valid, invalid or uncertain. The regenerator agent revises the uncertain entries based on the feedback provided by the evaluator. This self-refinement loop is repeated until a predefined stopping criterion is met, leaving entries that cannot be confidently accepted for expert review. The proposed framework is applied to the water processing and supply functional group of a 1 MW Proton Exchange Membrane (PEM) electrolyzer, focusing on the two components of hand valve HV-141 and centrifugal pump CW-281. The results obtained show that the method can automatically generate an accurate and complete FMEA draft, including both rows already identified by the expert and new safety-relevant rows that may have been overlooked in manual analyses due to time and effort constraints, while limiting the amount of expert intervention required for its validation.
L'analisi dei modi di guasto e dei loro effetti (Failure Mode and Effects Analysis, FMEA) è un metodo ampiamente utilizzato a supporto dell'analisi del rischio e del processo decisionale in materia di sicurezza nei sistemi industriali, ma la sua applicazione richiede molto tempo, è costosa e dipende fortemente dal giudizio degli esperti. Per affrontare queste sfide, questo lavoro propone un framework di Intelligenza Artificiale (IA) agentica per generare e affinare sistematicamente le tabelle FMEA. Ogni campo della tabella FMEA viene generato da una tripletta dedicata di agenti generatore, valutatore e rigeneratore. L'agente generatore propone voci FMEA candidate utilizzando conoscenze specifiche del dominio e del sistema recuperate da una base di conoscenza curata attraverso la Retrieval Augmented Generation (RAG). L'agente valutatore valuta le voci generate confrontandole con le conoscenze disponibili sul dominio e sul sistema, classificandole come valide, non valide o incerte. L'agente rigeneratore rivede le voci incerte sulla base del feedback fornito dal valutatore. Questo ciclo di auto-affinamento viene ripetuto fino al raggiungimento di un criterio di arresto predefinito, lasciando alla revisione degli esperti le voci che non possono essere accettate con sufficiente sicurezza. Il framework proposto è applicato al gruppo funzionale di trattamento e distribuzione dell'acqua di un elettrolizzatore PEM (Proton Exchange Membrane) da 1 MW, concentrandosi sui due componenti costituiti dalla valvola manuale HV-141 e dalla pompa centrifuga CW-281. I risultati ottenuti mostrano che il metodo è in grado di generare automaticamente una bozza di FMEA accurata e completa, comprendente sia le righe già identificate dall'esperto sia nuove righe rilevanti per la sicurezza che potrebbero essere state trascurate nelle analisi manuali a causa di vincoli di tempo e impegno, limitando al contempo la quantità di intervento umano necessaria per la sua validazione.
