The object of my work, which I’ll describe in the following pages, is an innovating technology with the purpose of automatizing prescriptions, distributions and management of drugs in hospitals, called Mario® (developed by Ingegneria Biomedica Santa Lucia S.p.a.). The first goal is the decision of a pool of parameters which allow performance evaluation for each system and the comparison among the three different versions which I observed and analyzed. Once parameters have been chosen, it’s necessary to define protocols for their evaluation or esteem. Those protocols will include the execution of experimental tests in loco and the analysis of Mario®’s database to assess their past performance. The tests results will be then exposed with particular emphasis on the differences caused by hardware upgrades from the oldest version to the newest ones, or by eventual anomalies. An optimal value for each parameter will then be set. The next step consists in investigating the reason of performance gaps occurring between the three Mario®s, and the elaboration and actuation of improvement protocols for the ones with the poorest results. Finally, tests will be repeated to assess the impact of the modifications, by comparing the before/after parameter values, and this will come with suggestions about the use of the system in order to avoid critical situations and anomalies. Everything will be introduced by two chapters describing what was the drugs iter from the pharmacy to the patient’s bed before automatization and informatization, with emphasis put on the critical factors which led to the introduction of those systems. This will be followed by a brief description of the main technologies used in Italy nowadays, among which figures Mario®.
L’oggetto del lavoro da me svolto, descritto in questo elaborato, è l’armadio robotizzato Mario®, tecnologia all’avanguardia nell’automazione delle prescrizioni, somministrazioni e gestioni delle terapie e dei farmaci ideato e realizzato dall’azienda Ingegneria Biomedica Santa Lucia S.p.A., con cui ho avuto l’occasione di collaborare. Il primo obiettivo è la scelta ragionata di un pool di parametri attraverso i quali è possibile caratterizzare e confrontare nel modo più completo possibile la performance delle tre differenti versioni dei Mario® che ho osservato ed analizzato. Una volta decisi i parametri, è necessario definire con precisione dei protocolli attraverso i quali è possibile ricavare o stimare i valori per i tre armadi. Questi protocolli prevederanno l’esecuzione di più test sperimentali in loco, nonché un’interrogazione dei database dei sistemi, contenenti dati storici sul loro funzionamento. I risultati saranno in seguito tabulati ed esposti in modo da sottolineare le differenze di prestazioni tra i Mario® causate dagli aggiornamenti hardware effettuati tra una versione e l’altra o da eventuali malfunzionamenti dei sistemi, definendo infine un valore ottimo di riferimento. Il passo successivo è l’indagine delle cause di scostamento degli armadi meno performanti da tale valore, e l’elaborazione con seguente messa in atto di protocolli di miglioramento volti ad eliminare per quanto possibile questo gap. Infine, saranno ripetuti i medesimi test effettuati a monte delle modifiche per quantificarne l’impatto sulle prestazioni dei sistemi e saranno proposti accorgimenti di cui tenere conto nell’utilizzo degli armadi da parte del personale volti a minimizzare imprevisti ed anomalie di funzionamento. Tutto ciò sarà introdotto da due capitoli che descrivono l’iter che i medicinali compievano per giungere dalla farmacia centrale al letto del paziente senza sistemi automatici enfatizzando le criticità che hanno portato alla loro introduzione, a cui segue una breve descrizione delle tecnologie attualmente più adottate nel nostro Paese tra cui, per l’appunto, figura l’armadio Mario®.
