While the effectiveness of carotid endarterectomy (CEA) from the symptomatic point of view has been largely demonstrated in several clinical trials, the effects of this procedure on local fluid dynamics are still poorly understood. Hence, a comparison of the fluid dynamics before and after CEA is mandatory, as it could provide useful information about the effectiveness of this surgical operation and a deeper understanding of the mechanism underlying atherosclerosis. In this context, aim of this thesis was to provide a quantitative comparison of the fluid dynamics before and after CEA by means of computational tools. In particular, the benefits due to CEA in terms of reduction of the risk of plaque rupture and reduction of abnormal flow patterns in the arterial bulk flow were investigated.
Mentre l’efficacia dell’endoarteriectomia carotidea (CEA) dal punto di vista della sintomatologia è stata dimostrata in numerosi trial clinici, gli effetti di tale procedura sulla fluidodinamica locale sono ancora poco compresi. Per questo motivo, un confronto della fluidodinamica prima e dopo la CEA è indispensabile, in quanto potrebbe fornire informazioni utili circa l’efficacia di questa operazione chirurgica e una più profonda comprensione dei meccanismi che stanno alla base dell’aterosclerosi. In questo contesto, obiettivo di questa tesi è stato quello di fornire un confronto quantitativo della fluidodinamica prima e dopo la CEA con l’utilizzo di strumenti computazionali. In particolare, sono stati investigati i benefici dovuti alla CEA in termini di riduzione del rischio di rottura della placca e di riduzione di pattern anomali del flusso sanguigno.
