Ultrasound represents one of the most widely used diagnostic technologies in the world. Among the reasons for its success, we could mention its immediacy of use, its stability and the fact that there is no risk for the patient. On the contrary, the professionals who practice ultrasound, i.e. sonographers, are often victims of musculoskeletal problems (or Work-Related Musculoskeletal Disorders, WRMSDs) due to the extended use of ultrasound equipment. Furthermore, ultrasound is subject to high variability due to its high dependence on sonographer expertise, patient body structure, and decubitus position. This thesis investigates the problem of WRMSDs and variability with the goal of arriving at the definition of a comprehensive solution. Applying design oriented methodologies, such as rapid prototyping and user testing, and completely rethinking the human-machine interaction of the ultrasound equipment, the ideation of SISTEMA (Supervised Imaging-US System, Two-controllers Ergonomic and Measuring Assistant) has been achieved. SISTEMA consists of three main components: a controller, a probe and an augmented reality (AR) headset. Through these three elements, with the addition of an external computational unit, it is possible to perform an ultrasound examination without burdening the posture of the sonographer. The sonographer, in fact, will always have within his field of view both the patient and the necessary information in addition to the commands for adjusting the ultrasound image. Thanks to computer vision, the use of augmented reality holograms and pressure and movement sensors installed inside the probe, it is possible to assist the sonographer in the execution of the examination. In this way, there is a validation by artificial intelligence (AI), regarding the execution of the examination. Due to the COVID-19 emergency, it was impossible to proceed to an in-depth user testing phase. Nevertheless, the concept has been validated remotely by a board of sonographers and medical experts. In addition, the ergonomic aspect has been submitted to various users, non-specialized, in order to validate the ergonomics. It would be optimal to carry out, in the future, an in-depth user testing phase together with sonographers and with an advanced prototype.
L’ecografia rappresenta una delle tecnologie di diagnostica più utilizzate al mondo. Tra i motivi del suo successo, abbiamo l’immediatezza d’uso, la stabilità e il fatto che non vi è alcun rischio per il paziente. Al contrario, i professionisti che praticano l’ecografia, cioè i sonographer, sono spesso vittima di problemi muscoscheletrici (o Work-Related Musculoskeletal Disorders, WRMSDs) dovuti all’utilizzo prolungato della attrezzatura ecografica. Inoltre, l’ecografia è soggetta a una elevata variabilità a causa della sua alta dipendenza dalla competenza del sonographer, dalla corporatura del paziente e dalla sua posizione di decubito. Questa tesi approfondisce il problema dei WRMSDs e della variabilità con l’obbiettivo di arrivare alla definizione di una soluzione omnicomprensiva. Applicando metodologie design oriented, come il rapid prototyping e l’user testing, e ripensando completamente l’interazione uomo-macchina della apparecchiatura ecografica, si è arrivati alla definizione di SISTEMA (Supervised Imaging-US System, Two-controllers Ergonomic and Measuring Assistant). SISTEMA è costituito da tre componenti principali: un controller, una sonda e un visore di realtà aumentata. Tramite questi tre è elementi, con l’aggiunta di una unità computazionale esterna, è possibile eseguire un esame ecografico senza gravare sulla postura del sonographer. Questo, infatti, avrà sempre all’interno del suo campo visivo sia il paziente che le infomrazioni necessarie oltre ai comandi per il controllo degli ultrasuoni. Grazie alla computer vision, all’utilizzo di ologrammi in realtà aumentata e a sensori di pressione e movimento applicati all’interno della sonda, è possibile ausiliare l’esecuzione da parte del sonographer. In questo modo si ha una validazione da parte della inteligenza artificiale, riguardo all’esecuzione dell’esame. A causa dell’emergenza COVID-19 è stato impossibile procedere a una fase di user testing approfondita. Nonostante questo il concept è stato validato a distanza da una board di sonographer e medici esperti del settore. Inoltre, l’aspetto ergonomico è stato sottoposto a vari utenti, non specializzati, in modo da poter validarne l’ergonomia. Sarebbe ottimale poter svolgere, in futuro, una fase di user testing approfondita assieme a dei sonographer e con un prototipo in fase avanzata.
